The oil and gas industry has seen some horrific incidents in its existence. Learning from these incidents help us understand what went wrong as well as figure out what could have been done differently. 1. Piper Alpha Disaster, North Sea, UK. On March 18, an oil spill occurred due to a valve leak, in Watford City, North Dakota. The Usumacinta blowout occurred in the Gulf of Mexico in 2007. An incoming storm caused the oil rig to collide with a wellhead, leading to an oil and gas leak. The platform eventually caught fire, and, despite an attempted evacuation by lifeboat, 22 workers died because of the accident.
